 1. Fixed \marginpar argument order
 2. Fixed handling of \extendmargin across pages
 3. Recovered lost padding between deferred notes
 4. Stop swallowing stretchiness of output page
 5. Add \topskip to deferred notes at top of new page
 6. Add margin phantoms with page-level optimization of positions
