 - Fix bug with vertical space before sectioning command in
   optional argument of \pstart

 - Fix bug when using \markboth on \doinsidelineLhook and
   \doinsidelineRhook. For people who previously used these
   commands, please tell me if the update breaks it
   (it should not)
