Following options are added:

1. "nopreprint" option to switch off preprint line.

2. "doubleblind" option added to typeset the PDF for double-blind
submission. 

3. "numafflabel" option to use numbers instead of alphabets
for author-affiliation linking. 

4. "flushleft" option to align title, author and affiliation to the
left side. 

5. A bug due to clash with cleveref.sty has been corrected.
