  Extensive improvements for indexing and glossaries.

  Added support for makeindex.  (Previously xindy only.)

  Added indexing packages: imakeidx, index, repeatindex, splitidx.

  Added improved automation for lwarpmk generation of indexing and glossaries.

  Added PDF-inclusion packages attachfile, attachfile2, intopdf, pdfpages, pdfx.

  Added package cases.

  Tested to work as-is with hvindex, notes2bib.
