 Changes
 -------

 - Fix - Misplaced R in page_precedence.

 - Fix - The last item was discarded.

 - \indexspecial, for literal unformatted entries.

 - delim_n now set with a macro (\indexpagessep).

 - gindex.ist does not add headings anymore. A new style
   gindexh.ist does.
