Changes since the last version:
- Add \crefthename, which is easier to use than the previous \crefthemark, and
with which \crefthe can now place the hyperlinks in the right place
- Add uppercased version \Crefthe and \Crefthename, which can do automatic case
changing for your convenience
- minor fixes and improvements
