Summary description:  A range of footnote options

 Fix an issue in the perpage package (gh/03)

 Footnotes on chapter pages in report/book class started with 2 when
 the perpage option was used.
 6.0c fixed the issue.
 6.0d makes some further adjustment to the fix in order to also support
 \MakePerPage[<startvalue>]{footnote} in case that is used in the document.
 
 This should really be fixed in the perpage package, but for now we
 do it here.
