 - Structure TDS-style zips correctly when files are
   to be used directly rather than extracted from sources
 - Better treatment of README files
 - Include MakeIndex styles in TDS-style zip construction
 - New approach to handling custom format requirements
 - Better handling of secondary files between test runs
 - Normalise upper half of 8-bit range when log file
   is in Unicode form
